The Routefinder is a way to help people to find the best route to the support needed to meet their needs.
It’s a series of documents that helps people to think about what they want to happen (their outcomes) and how to reach them.
The Routefinder is a valuable information sharing tool that can help to support the assessment process. It is written from the perspective of the customer and is owned by them. We hope that the Routefinder approach will replace traditional forms of assessment, and can be the primary means of gathering information about the support that customers need.
